Enclomiphene vs Testosterone Replacement Therapy (TRT)

TRT delivers synthetic testosterone, some of which converts to estrogen. Elevated estrogen suppresses LH and FSH via negative feedback, reducing the body’s own testosterone production and impairing sperm development. Enclomiphene avoids this issue by stimulating natural hormone release, preserving both testosterone production and sperm maturation

The hypothalamus releases gonadotropin-releasing hormone (GnRH), which stimulates the pituitary gland to secrete luteinizing hormone (LH) and follicle-stimulating hormone (FSH).

LH stimulates Leydig cells in the testes to produce testosterone. FSH supports sperm maturation.

Estrogen normally inhibits the release of LH and FSH through negative feedback. As a selective estrogen receptor modulator (SERM), enclomiphene blocks this estrogenic feedback, leading to increased LH and FSH levels and thus greater testosterone production and maintained fertility.
